Role Overview:

As a Home Health Nurse at BrightStar Care of W.Voluisa, you will be responsible for providing high-quality nursing care to patients in their homes. In this role, you’ll collaborate closely with our caregivers and healthcare team to ensure that our clients receive the best possible care tailored to their specific needs. Whether you work on-site in Jacksonville, you’ll have the opportunity to make a meaningful impact in the lives of those you serve.

Schedule/ Shifts Available Mon. - Thurs. = 11am - 8pm

Friday = 9am - 8pm

Saturday/Sundays = 9am - 7pm

Key Responsibilities:

1. Administer nursing care and medications as prescribed to patients in a home setting.
2. Assess patient's health status and provide updates to the healthcare team.
3. Educate patients and their families on health management and care plans.
4. Document clinical notes and monitor patient progress accurately.
5. Collaborate with caregivers to ensure comprehensive and patient-centered care delivery.

Qualifications:

You're a great fit if you have the following skills.

1. Active Registered Nurse (RN) license in the state of Florida.
2. Previous experience in home health care or a similar setting.
3. Strong communication skills and ability to work independently.
4. Compassionate demeanor with a dedication to patient care.
5. Ability to manage time effectively and prioritize tasks.

Bonus If You:

1. Have experience with dementia care or other specialized patient populations.
2. Hold certifications in CPR or First Aid.
3. Are fluent in additional languages besides English.

Employment Type & Availability:

This position is Full-Time, Monday through Friday.

Location:

We're looking for someone near Jacksonville, Florida, as this is an on-site position.
