Position Summary

BrightStar Care is seeking an experienced and compassionate Infusion Home Health Registered Nurse (RN) to provide skilled infusion therapy and nursing care to patients in the comfort of their homes. The Infusion RN is responsible for administering IV medications, monitoring patient responses to treatment, managing vascular access devices, performing patient assessments, coordinating care with providers, and educating patients and caregivers on infusion therapies and disease management.

The ideal candidate is clinically skilled, detail-oriented, and committed to providing exceptional patient-centered care.

Key Responsibilities

Infusion Therapy

Administer IV medications, hydration therapy, antibiotics, biologics, immunoglobulins, and other prescribed infusion treatments.

Perform pre-infusion, ongoing, and post-infusion patient assessments.

Monitor patients for adverse reactions and intervene appropriately as needed.

Ensure safe medication handling, preparation, administration, and disposal practices.

Vascular Access Management

Access, maintain, and troubleshoot:

PICC lines

Central venous catheters

Implanted ports

Peripheral IVs

Perform dressing changes and line maintenance according to established protocols.

Identify and report signs of infection, infiltration, occlusion, or other complications.

Clinical Assessment & Patient Care

Complete comprehensive nursing assessments and ongoing evaluations.

Monitor vital signs and patient response to treatment.

Communicate significant findings and changes in condition to physicians and interdisciplinary team members.

Develop and implement individualized plans of care.

Care Coordination

Coordinate care with physicians, pharmacists, case managers, and other healthcare professionals.

Ensure timely communication regarding therapy changes, lab results, and patient status.

Assist with transitions of care and discharge planning as appropriate.

Laboratory Services

Obtain ordered laboratory specimens through venipuncture or vascular access devices.

Process and document laboratory collections according to agency procedures.

Monitor and report relevant laboratory results.

Patient & Caregiver Education

Educate patients and caregivers regarding:

Infusion therapy administration

Medication purpose and side effects

Central line care

Infection prevention

Emergency procedures

Disease management

Reinforce physician and pharmacy instructions to support treatment compliance.

Documentation & Compliance

Complete accurate and timely clinical documentation.

Maintain compliance with agency policies, nursing standards, Medicare guidelines, and applicable state regulations.

Adhere to HIPAA requirements and patient confidentiality standards.

Qualifications

Required

Current Texas Registered Nurse (RN) license in good standing.

Current CPR/BLS certification.

Minimum one year of nursing experience.

Experience with IV therapy and vascular access management.

Strong assessment, communication, and organizational skills.

Valid driver's license, reliable transportation, and current auto insurance.

Preferred

Home health experience.

Infusion nursing experience.

Experience with PICC lines, ports, and central venous catheters.

CRNI certification or infusion-related specialty training.

Familiarity with EMR documentation systems.

Work Location: On the road / Patient homes throughout the service area
