Position Summary
BrightStar Care is seeking a compassionate, skilled, and dependable Home Health Registered Nurse (RN) to provide high-quality care to patients in their homes. The Home Health RN is responsible for conducting comprehensive assessments, Start of Care visits, care coordination, patient and caregiver education, wound care management, laboratory specimen collection, and collaboration with physicians and interdisciplinary team members to ensure optimal patient outcomes.
This role is ideal for a nurse who enjoys autonomy, building relationships with patients and families, and delivering exceptional care in the home setting.
Key Responsibilities
Clinical Care & Assessment
Complete Start of Care (SOC), Recertification, Resumption of Care, Evaluation, and Discharge visits.
Perform comprehensive nursing assessments and develop individualized plans of care.
Monitor patient progress and communicate changes in condition to the physician and care team.
Identify patient needs and coordinate appropriate home health services.
Care Coordination
Coordinate care with physicians, therapists, patients, caregivers, and community resources.
Collaborate with interdisciplinary team members to ensure continuity of care.
Participate in case conferences and care plan updates.
Therapy Evaluations & Referrals
Assess patients for home health services eligibility.
Identify needs for Physical Therapy (PT), Occupational Therapy (OT), and Speech Therapy (ST).
Coordinate therapy evaluations and ongoing services as appropriate.
Skilled Nursing Services
Perform routine and specialized nursing procedures.
Provide generalized wound care and wound assessments.
Collect laboratory specimens and perform lab draws as ordered.
Administer medications and monitor treatment effectiveness.
Manage chronic disease conditions and post-hospital recovery care.
Patient & Caregiver Education
Teach and train patients and caregivers regarding:
Disease processes
Medication management
Wound care
Safety and fall prevention
Infection control
Self-management strategies
Equipment and medical device use
Promote patient independence and improved health outcomes.
Documentation & Compliance
Complete accurate and timely electronic documentation.
Ensure compliance with home health regulations, agency policies, and nursing standards.
Maintain HIPAA compliance and patient confidentiality.
Qualifications
Required
Current Texas Registered Nurse (RN) license in good standing.
Current CPR/BLS certification.
Minimum one year of nursing experience.
Strong assessment, organization, and communication skills.
Valid driver's license, reliable transportation, and current auto insurance.
Preferred
Previous home health experience.
Experience completing Start of Care assessments.
Wound care experience.
Knowledge of Medicare home health guidelines.
Experience with electronic medical records (EMR).
