LPN – Skilled Home Care

Role Overview:

As a LPN – Skilled Home Care at BrightStar Care, you will be responsible for providing high-quality, personalized care to patients in the comfort of their own homes. In this role, you’ll collaborate closely with our supportive clinical and office team to ensure the well-being and comfort of our clients. Whether on-site in Athens, Texas, you’ll have the opportunity to build meaningful relationships with clients and their families through one-on-one patient care.

Key Responsibilities:

1. Providing skilled nursing care in the home setting.
2. Administering medications and treatments as prescribed.
3. Monitoring and documenting patient conditions accurately.
4. Collaborating with physicians, families, and care teams.
5. Following individualized care plans to ensure patient satisfaction.
6. Ensuring patient comfort, safety, and overall well-being.

Qualifications:

You're a great fit if you have the following skills.

1. Current Texas LVN license in good standing.
2. Strong clinical and communication skills.
3. Compassionate, dependable, and professional attitude.
4. Home health or private duty experience is preferred.

Bonus If You:

1. Have experience with specialized care (e.g., Alzheimer’s, dementia, etc.).
2. Possess additional certifications relevant to skilled nursing.

Employment Type & Availability:

This position is Full-Time, Monday through Friday.

Location:

This position is on-site only in Athens, Texas.
