RN Case Manager

Your Impact

• Discover the Whole Person: Conduct deep in-home assessments that capture not just medical histories, but the client’s passions, life stories, and what brings them joy.

Position Logistics

• Type: PRN
• Schedule: Monday through Friday daytime hours (protecting your personal work-life balance).
• Setting: Fully in-person / in-the-field.
• Location: Hilton Head Island, South Carolina.
