Local Home Care Partners is the locally owned company behind five BrightStar Care agencies across North and East Texas. Together, we provide a wide range of in-home services, including skilled nursing and personal care, to individuals and families throughout the region.

Role Overview

As a Licensed Vocational Nurse (LVN) – Skilled Home Care with BrightStar Care of Plano/North Dallas, you’ll provide compassionate, one-on-one skilled nursing care to clients in the comfort of their homes.

You’ll work closely with our supportive clinical and office teams to deliver individualized care while helping clients maintain their health, safety, comfort, and independence. This is an opportunity to build meaningful relationships with clients and families while making a direct impact every day.

This position is based in Fort Worth, Texas and requires in-home visits to clients throughout the service area.

Key Responsibilities

• Provide skilled nursing care in accordance with physician orders and individualized plans of care.
• Administer medications and treatments as prescribed.
• Monitor and assess clients’ conditions and report changes appropriately.
• Accurately document care provided and maintain timely clinical records.
• Follow established care plans and clinical protocols.
• Communicate effectively with physicians, clients, families, caregivers, and the BrightStar Care clinical team.
• Promote client safety, comfort, dignity, and overall well-being.
• Provide education and support to clients and their families as appropriate.

Qualifications

You may be a great fit for this position if you have:

• A current Texas Licensed Vocational Nurse (LVN) license in good standing.

• Strong communication and interpersonal skills.
• The ability to work independently while remaining connected to a supportive clinical team.
• Strong attention to detail and the ability to follow individualized plans of care.
• Reliability, professionalism, and a commitment to quality patient care.
• Flexibility to work a variety of shifts based on client and scheduling needs.

Bonus If You Have

• Previous home health, home care, or private-duty nursing experience.
• Current CPR and First Aid certification.
• Experience with specialized clinical skills such as wound care, IV therapy, or other complex nursing needs.

Employment Type & Availability

Part-Time

Flexible scheduling is available throughout the week, including Monday through Sunday, based on client needs and your availability.

Location

Fort Worth, Texas

This is an on-site, in-home position requiring travel to clients’ homes within the service area.
