Role Overview:

As a Licensed Practical Nurse at Holly Residential Care Center, you will be responsible for providing personalized nursing care and support to residents with Traumatic Brain Injuries and other significant physical and cognitive impairments. In this role, you'll collaborate closely with the nursing team to ensure the delivery of individualized care plans, and contribute to maintaining a warm and inclusive environment for both residents and staff.

Key Responsibilities:

  1. Provide direct care and medication administration to residents in accordance with their personalized care plans.
  2. Participate in the development and implementation of personalized, person-centered care plans.
  3. Collaborate with the nursing team to ensure the highest quality of nursing care and support is provided.
  4. Maintain accurate and timely documentation of residents' health status and care provided.
  5. Contribute to creating a warm and inclusive environment by engaging with residents and their families.

Qualifications:

You're a great fit if you have the following skills:

  1. Active Licensed Practical Nurse (LPN) license in the state of Oregon.
  2. Strong ability to provide compassionate, person-centered care to individuals with complex needs.
  3. Excellent communication and interpersonal skills to engage with residents and their families effectively.
  4. Ability to work collaboratively in a multidisciplinary team environment.
  5. Experience working with individuals with Traumatic Brain Injuries or similar conditions is a plus.

Bonus If You:

  1. Hold certifications in specialized nursing care for individuals with complex needs.
  2. Have experience in creating and implementing individualized care plans.
  3. Demonstrate fluency in languages spoken by the residents, including but not limited to English.

Employment Type & Availability:

This position is available as Full-Time or Part-Time and may include weekends.

We're looking for someone near the Eugene, Oregon area as this is an on-site position.
