Job Summary

The Intake and Outreach Manager will be responsible for operating entity financial growth, diversification and growth of payor types, and industry-compliant operations. This role will be evaluated on profitability (as measured by an increase in hours of service and diversification of revenue sources) and consistently compliant practices.

Key Responsibilities

  • Lead expansion efforts, with a focus on healthy payor mix and diversification – especially private pay market growth
  • Grow business, focusing on existing market expansion, while developing a robust pipeline of referral partners.
  • Serve as an ambassador for the Company, networking with referral sources to procure new revenue streams, including APD Case Managers, Social Workers, Nursing Facilities, VA Case Managers, Hospital Discharge Planners, Hospice Administrators, Urgent/Emergency Care Physicians, Insurance Brokers, Attorneys, etc.
  • Network with community engagement organizations such as Senior Brokers, Rotary and the Chamber of Commerce.
  • Document, monitor, analyze, track, update, and report on the generation of new revenue sources using CRM.
  • Intake new Clients during the initial in-home consultation.
  • Carefully track leads, providing documentation in Care Management Platform.
  • Manage relationships with both Prospect and Current Clients and their families.
  • Maximize profitability and growth while adhering to Oregon Administrative Rules, Federal, and State laws

Skills and Qualifications

  • Excellent verbal and written skills in English required.
  • Proficient in Excel, Microsoft Outlook, PowerPoint, and Word.
  • Excellent attention to detail and ability to maintain accurate records.
  • Thorough knowledge of best practices for handling PHI as governed by HIPAA.
  • Experience handling highly confidential and sensitive information.
  • Outstanding attention to detail, project management, documentation, organizational, and planning skills.
  • Ability to take initiative and work independently.
  • Ability to work in a fast-paced team environment with a commitment to reach goals and meet deadlines.
  • Exceptional time management abilities to meet deadlines and manage multiple tasks simultaneously.
  • Outstanding positive attitude with the ability to grasp and embrace the company's overall goals and objectives and portray them to customers and staff. Able to work well with others in a collaborative and respectful manner.

Education and Experience

  • Education – Bachelor’s Degree in Business, Marketing, or a related field preferred.
  • Experience – At least 5 years of experience in business development, marketing, or relationship management roles preferred.

Working Conditions

  • Most work is performed in a general office environment. Exposure to low to moderate levels of noise in a well-lit, well-ventilated, and fast-paced environment.
  • Requires manual/finger dexterity as necessary to perform daily job duties.
  • Occasionally required to reach with hands and arms; lift, push, pull, and/or carry objects up to 35 pounds.
  • Frequently required to sit for extended periods of time.
  • Frequent and/or continuous talking.
  • Prolonged periods of standing and moving.
  • Must be able to lift up to 50 pounds at times with or without assistance.

Work Hours: Monday – Friday, 8am – 5pm

Supervisory Responsibilities: No

Required Travel: Up to 100%
