LPN – Skilled Home Care

Role Overview:

As an LPN – Skilled Home Care at BrightStar Care, you will be responsible for delivering high-quality nursing care to clients in their homes. In this role, you’ll collaborate closely with our multi-disciplinary team to ensure seamless patient care and support. Whether you work on-site in Fort Wayne, Indiana, you’ll have the flexibility to manage your schedule within our operational hours.

Key Responsibilities:

1. Administer medications and treatments as prescribed by physician instructions via G-tube and trach.
2. Monitor and assess patient health, documenting any changes or concerns.
3. Provide education and support to patients and their families regarding health management.
4. Assist with personal care activities, including bathing and feeding.
5. Collaborate with nursing staff and other healthcare providers to ensure comprehensive care plans are followed. Take ownership of patient assessments and help drive positive health outcomes.

Qualifications:

You're a great fit if you have the following skills.

1. Valid LPN licensure in Indiana.
2. Experience in home health care or skilled nursing.
3. Strong communication and interpersonal skills.
4. Ability to work independently and manage time effectively.
5. Knowledge of electronic medical records (EMR) systems.

Employment Type & Availability:

This position is Full-Time, available seven days a week.

Location:

This position is on-site only in Fort Wayne, Indiana.
