Registered Nurse, Home Health, Part Time Flexible

Responsibilities

Perform in-home nursing assessments and create individualized care plans
Conducts all certifications & recertifications using OASIS document system
Provide skilled nursing care such as wound care, medication management, injections, catheter care, and chronic disease monitoring
Educate clients and families on care plans, medication regimens, and health maintenance
Supervise and support Home Health Aides to ensure care plan adherence and excellent service
Maintain thorough and timely clinical documentation
Communicate with physicians, case managers, and office staff regarding client needs and changes in condition
Participate in case conferences and quality improvement activities as needed

Qualifications

Active Florida RN license (required)
Minimum 1 year of home health nursing experience
Valid driver’s license and reliable transportation
Strong clinical skills and sound judgment
Excellent communication and interpersonal skills
Ability to work independently in the field while remaining connected with the care team
