The Opportunity

As an LVN – Skilled Home Care, you’ll provide one-on-one skilled nursing care to patients in the comfort of their homes. This is an opportunity to build meaningful relationships with your patients and their families while providing personalized care tailored to each individual’s needs.

You’ll work closely with the patient, family members, physicians, and BrightStar Care’s clinical team to promote safety, comfort, and positive outcomes.

This is a part-time position with flexible scheduling opportunities, including weekdays and weekends.

What You’ll Do

• Provide skilled nursing care in the patient’s home according to the plan of care.
• Administer medications and treatments as ordered.
• Monitor patient conditions and report changes appropriately.
• Accurately document patient care and clinical observations.
• Follow physician orders and individualized plans of care.
• Communicate effectively with patients, families, physicians, and the BrightStar Care clinical team.
• Promote patient comfort, dignity, safety, and independence.
• Provide compassionate, reliable, and professional care during every visit.

What We’re Looking For

• Current Texas LVN license in good standing.
• Strong clinical skills and attention to detail.
• Excellent communication and interpersonal skills.
• Compassionate, dependable, and patient-focused approach to care.
• Reliable transportation for patient visits.
• Current CPR certification preferred.
• Home health or private duty nursing experience preferred, but not required.

You’ll Stand Out If You Have

• Previous experience providing care in a home setting.
• Experience working with medically complex patients.
• Additional certifications or specialized clinical experience.
• A passion for building relationships with patients and their families.
