We are seeking a compassionate and skilled Registered Nurse (RN) to join our team. This position is ideal for nurses who value flexibility and want to make a direct impact by providing high-quality, short-term care for patients while giving their primary caregivers much-needed relief.

As an RN, you’ll deliver personalized, hands-on nursing care in a supportive home-based environment—helping patients feel safe, comfortable, and cared for during transitional periods.

What You’ll Do

Direct Patient Care

• Provide high-quality nursing care, including medication administration, vital sign monitoring, and wound care.
• Observe, assess, and respond to changes in patient condition.

Assessment & Care Planning

• Conduct comprehensive patient assessments.
• Develop and implement individualized care plans in collaboration with the healthcare team.

Documentation

• Accurately document all care provided, assessments, and changes in condition using electronic health records (EHR).

Collaboration

• Work closely with interdisciplinary team members to ensure coordinated and comprehensive patient care.

Family & Caregiver Support

• Educate and support patients and families, answering questions and offering guidance on care practices.

Emergency Response

• Respond promptly and effectively to medical emergencies or sudden changes in patient condition.

Compliance & Safety

• Adhere to all company policies, procedures, and regulatory requirements, including infection control and safety protocols.

Flexibility

• Adapt to varying schedules and assignments based on patient needs.

Qualifications

• Active Indiana RN License
• Minimum 1 year of nursing experience (Required)
