Your Impact

Evaluate clients and develop individualized treatment plans based on physician's orders
Provide in-home physical therapy services focused on mobility, strength, and functional independence
Educate clients and families on exercises, safety, and injury prevention
Document evaluations, progress notes, and outcomes accurately and timely
Collaborate with nurses, caregivers, and care coordinators to support the plan of care
Uphold all state, federal, and agency compliance standards

Qualifications

Current Washington State Physical Therapist license (required)
Graduate of an accredited Physical Therapy program
Previous home health or outpatient experience preferred (but not required)
Strong communication and clinical judgment skills
Ability to work independently while remaining connected to a team
Reliable transportation and ability to travel to client homes
