BrightStar Care is seeking dependable Home Health Registered Nurse (RN) to support our growing patient needs throughout the Fort Worth area. We are looking for nurses who are flexible, available to accept visits, and experienced in providing quality home health care.

Areas of Need

• Start of Care (SOC) Visits
• Wound Care
• Therapy Start of Care (SOC) Visits

Responsibilities

• Complete comprehensive patient assessments and Start of Care visits.
• Provide wound care services according to physician orders and established care plans.
• Complete therapy-related SOC visits as appropriate to your qualifications and assigned responsibilities.
• Develop and update patient care plans in collaboration with the healthcare team.
• Document visits accurately and submit required clinical paperwork promptly.
• Communicate patient changes and concerns to the appropriate clinical team members.
• Deliver compassionate, patient-centered care in the home health setting.

Qualifications

• Active, unrestricted Texas Registered Nurse (RN) license.
• Home health experience preferred, with relevant experience in SOC assessments and wound care.
• Strong clinical assessment, documentation, and communication skills.
• Ability to work independently and manage visits throughout the Fort Worth service area.
• Reliable transportation and willingness to travel between patient homes.
• Must be available and willing to accept visits based on patient needs.
