As an RN/LVN, you will provide high-quality nursing care to patients in the comfort of their own homes while working closely with patients, families, and our clinical team.

What You'll Do

• Provide skilled home health and private-duty nursing services according to each patient's individualized Plan of Care.
• Perform nursing services within the scope of the Texas Nurse Practice Act.
• Provide specialized nursing care, including infusion therapy and other skilled services as assigned.
• Monitor patients' conditions and promptly report changes to the appropriate clinical personnel.
• Perform wound care, lab draws, PICC care, and other skilled nursing procedures within your scope of practice.
• Maintain accurate and timely clinical documentation in compliance with local, state, and federal regulations.
• Educate and counsel patients and families regarding their healthcare and nursing needs.
• Promote patient safety, comfort, dignity, and quality of life.
• Ensure the quality and adequacy of services provided.
• Be available to provide both patient visits and shifts based on client needs and your availability.
• Travel to patient locations throughout the Tyler and surrounding East Texas service areas.

What You'll Need

• Current, unrestricted RN or LVN license in the state of Texas.
• 2–5 years of infusion experience preferred.
• Infusion therapy experience strongly preferred.
• Wound care experience.
• Lab draw and PICC care experience.
• Port experience is a plus.
• Current CPR/BLS certification.
• Recent TB test or chest X-ray, within the last year.
• Reliable transportation, valid driver's license, and current auto insurance.
• Ability to pass a pre-employment drug screening.
• Ability to pass required state and federal background checks.
• Proof of eligibility to work in the United States.
• Willingness to travel to patient locations as assigned.
