Overview: The Client Care Coordinator at Homewatch CareGivers Cincinnati plays a vital role in ensuring a seamless and compassionate experience for clients and their families. This position is responsible for managing client intake, developing care plans, coordinating caregiver assignments, and maintaining high levels of client satisfaction. As one of the primary liaisons between clients, families, caregivers, and internal teams, the Client Care Coordinator ensures that care delivery meets both regulatory standards and the personal needs of each client.
Key Responsibilities:
- Part-time CareGiver and fill-in as needed
- Conduct comprehensive client assessments to determine care needs, preferences, and medical conditions.
- Develop and maintain individualized care plans in collaboration with clients, families, and healthcare providers.
- Update care plans monthly as client conditions or preferences evolve.
- Educate families on available services, expectations, and care procedures.
- Serve as a point of contact for clients and families, providing updates and resolving concerns.
- Conduct regular client check-ins and satisfaction surveys.
- Address complaints or service issues promptly and professionally.
- Maintain strong relationships to foster trust and long-term engagement.
- Match caregivers to clients based on skills, experience, and personality fit.
- Collaborate with the Scheduling Coordinator to ensure adequate staffing and coverage.
- Work with our Scheduling Coordinator to manage scheduling conflicts, emergency coverage, and last-minute call-offs, be able to cover a shift for last minute call-offs.
- Monitor caregiver performance and provide feedback or coaching as needed.
- Assist in caregiver onboarding and training at the clients home to ensure alignment with client needs.
- Maintain accurate and timely documentation of care plans, service notes, and client records.
- Ensure compliance with HIPAA, state and federal regulations, and company policies.
- Support audits, licensing, and accreditation processes.
- Track client progress and caregiver performance for continuous improvement.
- Work with Community Outreach/Owner and Scheduling Coordinator to support client onboarding and transitions.
- Coordinate with healthcare providers, insurance representatives, and case managers to secure additional resources.
- Provide reports to leadership on client satisfaction, service trends, and operational effectiveness.
- Participate in quality assurance initiatives and internal team meetings.
- Support emergency response planning and crisis management for clients.
- Maintain knowledge of local resources and referral networks to support client needs.
- Promote a culture of compassion, professionalism, and excellence in care delivery.
- Other duties as assigned.
Qualifications:
- Bachelor’s degree in Healthcare Administration, Social Work, Nursing, or related field preferred.
- Minimum of 3-5 years experience in client coordination, case management, or home care administration.
- Strong interpersonal and communication skills; ability to engage effectively with diverse audiences.
- Experience with being a caregiver
- Experience with caregiver scheduling and client relationship management is a plus.
- Knowledge of HIPAA and home care industry standards is a plus.
- Ability to work independently and manage multiple priorities effectively.
- Proficient in using ERM tools and Microsoft Office Suite.
- Reliable transportation and willingness to travel within the Cincinnati, Ohio area.
Physical Demands:
- Ability to work flexible hours per week (20-25 hours and increasing to full-time)
- Capable of bending, climbing, stooping, and standing for up to 5 hours per day.
- Able to lift 20 pounds if needed.
- Must be able to use tools and equipment necessary for the role.
- Ability to travel to client homes and community locations as needed.
