What You’ll Do:
· Administer medications and treatments as prescribed by physicians.
· Conduct thorough health assessments and develop personalized care plans tailored to each client’s needs.
· Provide wound care, manage dressings, and monitor healing to ensure client safety and well-being.
· Educate clients and their families on managing their health conditions, medications, and nutrition.
· Collaborate with a multidisciplinary team to deliver comprehensive care, ensuring all health needs are addressed.
· Monitor vital signs and report changes to the care team promptly.
· Assist clients with daily living activities, including mobility, hygiene, and nutrition.
· Conduct quality assurance visits and new client assessments to maintain high care standards.
· Supervise and train caregivers and nursing staff, fostering adherence to regulations and quality care protocols.
· Maintain accurate and up-to-date documentation of all client care activities and progress.
Are You the Right Fit?
· Valid RN in the state of Ohio.
· Strong clinical assessment skills and knowledge of home care practices; experience with geriatrics is a plus.
· Excellent communication and interpersonal skills to effectively build rapport with clients and their families.
· Ability to work both independently and collaboratively within a dynamic team environment.
· Proficient in electronic health records (EHR) and other relevant healthcare technology.
· CPR and First Aid certification required; CPR Instructor certification is a bonus.
· Knowledge of Ohio Home Health regulations is strongly preferred.
· Reliable transportation and a willingness to travel.
