We partner with physicians, nurse practitioners, physician assistants, hospitals, and senior living communities to help patients recover safely, remain independent, and avoid unnecessary hospital readmissions.

We are seeking a compassionate, experienced Registered Nurse to provide clinical oversight, caregiver supervision, and care coordination for our growing private-duty home care team.

Responsibilities

• Conduct initial in-home nursing assessments and develop individualized plans of care.
• Perform supervisory visits to ensure caregivers are providing safe, high-quality care.
• Complete caregiver competency evaluations and skills assessments.
• Coordinate care with physicians, hospitals, rehabilitation facilities, hospice providers, and assisted living communities.
• Identify changes in a client's condition and communicate recommendations promptly to the healthcare team.
• Educate clients and families on disease processes, fall prevention, medication safety, and home safety.
• Support post-acute patients transitioning home following hospitalization or rehabilitation.
• Participate in quality assurance, documentation review, and regulatory compliance.
• Serve as a clinical resource for caregivers and office staff.
• Build relationships with referral partners while promoting our commitment to exceptional patient care.

Qualifications

• Active, unrestricted North Carolina Registered Nurse (RN) license.
• Minimum of one year of clinical nursing experience; home care, hospice, geriatrics, care management, or transitional care experience preferred.
• Strong assessment, communication, organizational, and critical-thinking skills.
• Comfortable working independently in the community.
• Valid driver's license, reliable transportation, and current auto insurance.
• Proficient with electronic documentation and Microsoft Office.
