Role Overview:

The Homewatch CareGivers of San Rafael, Client Care Leader will oversee a growing team of Home Care Aides and ensure that patients receive high-quality, personalized care in the comfort of their homes through detailed care planning

What you can expect in your role:

• Conduct thorough assessments of clients’ needs and develop personalized care plans.
• Ensure clients' health conditions and changes are incorporated into the care plan and communicated to the care team.
• Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to coordinate comprehensive care.
• Supervise:
  • Home Care Aides and Client Care Scheduler
• Conduct quality assurance visits to ensure compliance with care standards.
• Supervise and train caregivers, ensuring adherence to state regulations and quality care protocols.
• Maintain accurate and timely documentation of client care activities and progress.

What differentiates you:

• Strong interpersonal and communication skills; ability to engage effectively with diverse audiences.
• Proven track record of successful relationship-building in a community or healthcare setting.
• Minimum of 4 years nursing experience, including at least 2 year(s) in home care or a closely related field.
• Strong clinical assessment skills and knowledge of home care practices; experience in geriatrics preferred.
• Ability to work independently and as part of a dynamic team.

We require a few things:

• CPR and First Aid certification required; CPR Instructor certification preferred.
• Knowledge of California Home Health regulations is strongly preferred.
• Valid driver’s license, a reliable car & auto insurance able to get to referral partners throughout Marin County
• Clean background check

Where will you be working?

• Our office is in San Rafael
• Our referral partners and clients are located throughout Marin County in the communities of San Rafael, Novato, Ross, San Anselmo, Kentfield, Greenbrae, Larkspur, Corte Madera, Mill Valley, Tiburon, Sausalito.
