Role Overview

As an LPN/LVN – Skilled Home Care with BrightStar Care of Plano/North Dallas, you’ll provide personalized, one-on-one skilled nursing care to patients in the comfort of their own homes.

This role is ideal for an experienced LVN who is confident managing wound vacs and enjoys building meaningful relationships with patients while working independently. You’ll have the support of a dedicated clinical team while maintaining the flexibility to work part-time around your availability.

Key Responsibilities

• Provide skilled nursing care in the patient’s home
• Manage, monitor, and provide care related to wound vac therapy
• Administer medications and treatments as ordered
• Monitor and assess the patient’s condition and report changes appropriately
• Maintain accurate and timely clinical documentation
• Follow the patient’s individualized plan of care and physician orders
• Communicate effectively with physicians, family members, and the BrightStar Care clinical team
• Promote patient comfort, safety, dignity, and overall well-being
• Provide compassionate, professional care while maintaining clinical standards

Qualifications

You’re a great fit for this position if you have:

• Current Texas LVN/LPN license in good standing
• Hands-on experience with wound vac management
• Skilled nursing or home care experience preferred
• Strong clinical assessment and communication skills
• Ability to work independently while remaining connected to the clinical team
• Excellent organization and documentation skills
• A compassionate, dependable, and patient-centered approach to care
• Reliable transportation, a valid driver’s license, and current auto insurance

Bonus If You Have

• Additional certifications related to home health or skilled nursing
• Experience caring for patients with complex medical needs
• Previous experience providing one-on-one care in a home setting

Employment Type & Schedule

Part-Time | Flexible Scheduling

Scheduling opportunities are available Monday through Sunday, with the ability to work shifts that fit your availability and our patients’ needs.

Location

Prosper, Texas

This is an in-home, field-based position serving patients in the Prosper area.
