Home Health Aide (HHA) – On Call

Location: Fort Wayne, IN

Pay: $18.00 - $23.00 per hour

Job Type: Full-time and Part-Time

Required Availability: One on-call day each week and one on-call weekend per month

BrightStar Care of Fort Wayne is recruiting an experienced Home Health Aide (HHA) or Certified Nursing Assistant (CNA) for our Set Team in Fort Wayne, IN.

This role focuses on helping cover caregiver absences and other staffing needs. You’ll bring confidence, adaptability, and dependable care to different clients while staying connected with our scheduling and clinical teams.

Required On-Call Availability

• One designated day each week, 6 a.m.–11 p.m.
• One designated weekend each month
• Remain reachable during your assigned on-call periods
• Be available to accept client coverage during those periods
• Coordinate your assignments with the scheduling team

Scheduling options will be discussed around these coverage requirements.

How You’ll Support Our Clients

Your Home Health Aide (HHA) or Certified Nursing Assistant (CNA) experience will help you step into assignments, understand each client’s routine, and provide attentive care.

• Review the care plan before providing services.
• Respond to coverage requests and confirm assignment details.
• Assist with bathing, dressing, grooming, and personal hygiene.
• Help clients move, transfer, and reposition safely.
• Perform prescribed mobility exercises according to the care plan.
• Prepare meals, assist with household tasks, and offer companionship.
• Help with transportation to appointments as assigned.
• Record services provided and communicate important updates.
• Notify the nurse case manager of changes in condition.
• Respect each client’s preferences, dignity, and confidentiality.

What You’ll Bring

• At least one year of home health experience as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA)
• Current Indiana HHA or CNA credentials appropriate to the assignment
• High school diploma or GED
• Dependability and willingness to work with different clients
• Comfort traveling to assigned homes in Fort Wayne, IN
• Valid driver’s license, auto insurance, and a reliable vehicle
• Physical exam and negative TB test within the past 12 months
• Ability to lift up to 50 pounds and assist with mobility
• Strong English reading, writing, and verbal communication skills
• Commitment to HIPAA compliance
• CPR certification preferred
